มี 3 ระยะได้แก่
- Preeruptive phase ปวด คันหรือ ชา 1-10 วัน
- Acute eruptive phase มี Grouped herpetiform vesicle on erythematous base ตาม dermatome ต่อมา rupture เป็น crust , erythematous plaques มีอาการปวดมาก รวมระยะเวลา 10-15 วัน ผื่นใช้เวลาเป็นเดือนจึงหาย
- Post herpetic neuralgia คือยังคงมีอาการปวด > 30 วันหลังจากที่ผื่นตกสะเก็ดแล้ว
Rx:
- pain control (เช่น topical NSAIDs/lidocaine, NSAIDs, nacrotic, gabapentin), calamine lotion (แก้คัน เป็น mild antisepticและช่วยซับ oozing)
- wet-to-dry dressing ด้วย NSS หรือ Burrow solution ทิ้งไว้ 30-60 นาที 4-6 ครั้ง/วัน
- Valacyclovir 1 gm PO q 8 h หรือ Famiciclovir 500 mg PO q 8h x 7 วัน ไม่จำกัดว่าต้องให้ใน 72 ชั่วโมง ได้ประโยชน์ใน immunocompromised หรืออายุ > 50 ปี
- +/- Prednisolone 40-60 mg OD เช้า x 1 wks then taper 1-2 wks ในกรณี moderate-severe pain หรือมี significant neurological symptoms โดยต้องให้รวมกับ antiviral
