Approach to low back pain

Back pain สามารถแบ่งตามระยะเวลาออกเป็น acute (< 6 สัปดาห์), subacute (6-12 สัปดาห์), chronic (> 12 สัปดาห์)

สามารถแบ่งออกเป็น

• Mechanical low back pain (congenital/degenerative, trauma) เช่น lumbar strain, spondylosis, facet joint pain, spondylolisthesis, herniated disc, spinal stenosis, fracture, scoliosis, kyphosis, lumbosacral transitional vertebra
• Non-mechanical spine disease (tumor, infection, inflammatory arthritis) เช่น multiple myeloma, metastatic cancer, lymphoma, leukemia, spinal cord tumor, retroperitoneal tumor, osteomyelitis, septic discitis, paraspinal abscess, epidural abscess, ankylosing spondylitis, psoriatic spondylitis, Reiter’s syndrome, inflammatory bowel disease
• Visceral disease (GI, GU, GYN, + Vascular) เช่น pancreatitis, cholecystitis, penetrating ulcer, nephrolithiasis, pyelonephritis, prostatitis, endometriosis, PID, aortic aneurysm

ซักประวัติ

• Onset, location, duration, characteristic, refer pain (radicular pain), aggravating factors (cough, Valsalva, sitting สงสัย herniation), relieving factor, previous treatment, associated symptoms (ไข้ เบื่ออาหาร น้ำหนักลด)
• ประวัติที่สำคัญ (tumor, infection) คือ night pain, unrelenting pain (ทำอย่างไรก็ไม่ดีขึ้น)
• แยกสาเหตุจาก mechanical และ non-mechanical back pain (ดูเรื่อง inflammatory back pain)
• ประวัติ complications (neurologic deficit) ได้แก่ อาการชา อ่อนแรง เดินลำบาก กลั้นปัสสาวะ-อุจจาระไม่ได้ (แต่ถ้าประวัติและตรวจร่างกายอย่างอื่นปกติ อาจทำ US หรือ post-void catheterization ถ้าปริมาณ urine > 100 mL แสดงว่ามี overflow incontinence)
• Red flag signs เพื่อหาสาเหตุที่สำคัญ ได้แก่ fracture, cancer, infection, neurological complication ได้แก่
• Fracture: อุบัติเหตุที่รุนแรงสัมพันธ์กับอายุ เช่น ล้มหรือยกของหนักในคนที่อาจจะมีกระดูกพรุน
• Cancer: CA with bone metastasis, unexplained weight loss
• Infection: ปวดรุนแรงและมีประวัติ lumbar spine surgery, IVDU, immunosuppression; มีไข้, UTI, มีแผลบริเวณกระดูกสันหลัง
• Cauda equine syndrome: bowel/bladder incontinence, urinary retention, major motor weakness, sensory deficit, loss of anal sphincter tone, saddle anesthesia

ตรวจร่างกาย

• ดู: signs ของ skin/soft tissue infection, trauma
• คลำ: pain on palpation หรือ percussion ที่ vertebral bodies ให้สงสัย fracture หรือ infection; กดตรง interspinous ligament แล้วขยับนิ้ว ถ้าปวดสงสัย disc; ถ้าขยับมากดด้านข้างออกมา 2.5 ซม. แล้วปวดสงสัย facet joint
• Straight leg raising test (หรือตรวจ seated straight leg test แต่ sensitivity ต่ำกว่า) ตรวจหา disk herniation ที่ L4-5 หรือ L5-S1 ให้นอนยกขาสูง 70^o แล้วมี radicular pain และจะปวดมากขึ้นเมื่อทำ ankle dorsiflexion และอาจตรวจ crossed SRLT (specific 90%)
• Neurological exam
• Sensation ตรวจ light touch ตามด้วย pinprick, temperature, proprioception และ vibration ตาม dermatome (L5 numbness ที่ medial foot, web space 1^st-2^nd toe; S1 numbness ที่ posterior calf, lateral foot)
• Motor: L4 ทำ ankle dorsiflexion; L5 ทำ extensor halluces longus dorsiflexion; S1/S2 ทำ ankle plantar flexion
• Reflex: L3/L4 ตรวจ patellar reflex, S1 ตรวจ Achilles reflex; Babinski’s reflex
• PR ดู rectal tone, perianal anesthesia เฉพาะในรายที่สงสัย
• Waddell’s signs ในคนที่มีปัญหา chronic pain และมี psychological distress อาจจะทำให้มีอาการมากผิดปกติ

Investigation

• ไม่จำเป็นต้องทำ imaging ในช่วง 4-6 สัปดาห์แรก ถ้าไม่มี neurological abnormality หรือ ไม่มี red flag sign ที่สำคัญ เช่น new onset pain with suspicious/risk factor for cancer, new onset pain with fever/IVDU/recent infection, suspected cauda equina syndrome
• MRI ในกรณีสงสัย infection, cauda equina syndrome, severe/progressive neurological deficit
• ESR เมื่อสงสัย infection หรือ inflammatory back pain

ในกรณีที่ทดลองรักษาแล้วอาการไม่ดีขึ้นใน 4-6 สัปดาห์

• Plain AP, lateral ในคนที่อาจจะมีความเสี่ยงต่อ cancer (อายุ > 50 ปี, unexplained weight loss), inflammatory back pain (ฝืดตึงตอนเช้า > 30 นาที ปวดก้นซ้ายขวาสลับไปมา ปวดตอนกลางคืน), vertebral compression fracture (osteoporosis, glucocorticoid use, significant trauma)
• MRI ในคนที่มี radiculopathy ที่อาจจะรักษาด้วยการฉีด steroid หรือ ผ่าตัด

Specific disease

• Acute nonspecific back pain มีอาการปวดแบบ mechanical back pain ขยับแล้วปวดมากขึ้น พักแล้วอาการดีขึ้น และไม่พบ risk factor; Tx: พยายามให้กลับมา normal activity หลีกเลี่ยงการบิด ก้มตัว ให้ยา paracetamol +/- NSAIDs, ยาอื่นๆ เช่น opioid (< 1 สัปดาห์), muscle relaxant (diazepam 5-10 mg PO q 6-8 h, methocarbamol 1000-1500 mg QID); warm compression, physical therapy (เช่น McKenzie method); ติดตามอาการช่วง 4-6 สัปดาห์ 
• Chronic nonspecific back pain: รักษายาก อาจให้ opioid ช่วงสั้นๆ (แต่ได้ประโยชน์ไม่มาก) ให้ review ประวัติว่าได้ R/O serious cause แล้วหรือไม่
• LBP with sciatica เรียกรวมโรคที่เกิดจาก spinal nerve, cauda equine หรือ spinal cord compression; สาเหตุ เช่น disk herniation (most common), intraspinal tumor/infection, foraminal stenosis, extraspinal plexus compression, piriformis syndrome, lumbar canal stenosis (spinal stenosis)
• Disk herniation อาการจะเด่นที่ radicular pain (ปวดร้าวเลยเข่า) > back pain ตำแหน่งที่พบบ่อยคือ L4-5 (L5 root), L5-S1 (S1 root) ตรวจพบ SRLT positive; Ix: plain film ในรายที่มี neurological deficit เพื่อหาสาเหตุอื่นๆ (เช่น tumor, infection, fracture, spondylolisthesis), MRI ในรายที่มี severe progressive neurological deficits และสงสัย serious cause; Tx: ให้กลับมา normal activity ให้ยาแก้ปวดตามอาการ ส่วนใหญ่อาการจะดีขึ้นภายใน 6 สัปดาห์
• Spinal stenosis พบในคนอายุ > 60 ปี มีอาการปวดเรื้อรัง +/- sciatica pain อาการเป็นมากเมื่อยืนนานๆ หรือ spinal extension อาการดีขึ้นเมื่อพักหรือเอนตัวมาด้านหน้า มีอาการปวดหลังและขาขณะเดิน (neurological claudication) ต้องแยกจาก vascular claudication; Ix: CT หรือ MRI; Tx: รักษาตามอาการ
• Ankylosing spondylitis: เป็น inflammatory back pain ลักษณะเด่นคือ ส่วนใหญ่พบในผู้ชาย (3:1) อายุน้อย (< 40 ปี) ปวดเรื้อรัง (> 3 เดือน) มีอาการฝืดตึงตอนเช้า (> 30 นาที) อาการดีขึ้นเมื่อขยับบริหารร่างกาย; Ix: Film พบ bamboo spine (sacroilitis, square vertebral bodies)
• Epidural compression syndrome (spinal cord compression, cauda equina syndrome, conus medullaris syndrome): สิ่งสำคัญคือต้อง detect ให้ได้ตั้งแต่ระยะแรก ประวัติที่สำคัญได้แก่ อาการชารอบรูก้น การกลั้นอุจจาระหรือปัสสาวะไม่ได้ (ถ้ามี urinary retention > 500 mL + (bilateral sciatica หรือ subjective urinary retention หรือ rectal incontinence) จะมีความจำเพาะต่อ cauda compression) การตรวจร่างกายที่สำคัญ ได้แก่ urinary retention, lower extremities weakness/paresthesia, gait difficulty, abnormal SLRT, saddle anesthesia (buttock, posterosuperior thighs, perineal), decrease anal tone; Tx เมื่อสงสัยให้ dexamethasone 10 mg IV ก่อนทำ emergent MRI whole spine (ยกเว้นสงสัย cauda equine syndrome จาก herniated disk อาจทำ MRI เฉพาะ lumbosacral spine)
• Transverse myelitis มาด้วยอาการเดียวกับ epidural compression syndrome อาการขึ้นกับระดับที่เกิด inflammation ตัวโรคจะพัฒนาขึ้นในเวลาหลายวันถึงหลายสัปดาห์; Ix: MRI อาจปกติได้ในช่วงแรก ตรวจ CSF จะพบ lymphocytosis, elevated protein; Tx: consult neurologist, ให้ corticosteroids, plasma exchange
• Spinal infection
• Vertebral osteomyelitis มีอาการปวดเรื้อรัง > 3 เดือน มีไข้ กดเจ็บ; Ix ESR, CRP เพิ่ม, film (ผิดปกติหลัง 2-8 สัปดาห์) อาจพบ bony destruction, vertebral end plate irregularity, disk space narrowing; เมื่อสงสัยภาวะนี้ให้ทำ H/C
• Discitis มีอาการปวดตลอดเวลา ปวดจนต้องตื่นกลางดึก ไม่ดีขึ้นเมื่อพักหรือกินยาแก้ปวด อาจมีไข้; Ix ESR เพิ่ม
• Spinal epidural abscess มี triad คือ severe back pain, fever, neurological deficit มักเป็นร่วมกับ vertebral osteomyelitis หรือ discitis
• Ix: MRI; Tx: consult spine surgeon, IV ATB (หลัง consult) ได้แก่ piperacillin-tazobactam 3.375 mg IV + vancomycin 1 gm IV
