Dry eye disease (DED)
- เกิดจาก 2 กลไก คือ
- Decrease tear production มีการสร้าง aqueous fluid จาก lacrimal gland ลดลง ทำให้เกิด tear
hyperosmolarity และเกิด ocular surface inflammation ตามมา สาเหตุ เช่น
Sjögren's syndrome, lacrimal ductal obstruction
(age-related DED, conjunctival scarring [trachoma, pemphigoid, vitamin
deficiency, post-viral syndrome, ocular burns])
- Increased evaporative loss ส่วนใหญ่เกิดจาก meibomian gland dysfunction (posterior blepharitis) ซึ่งทำให้ lipid layer ของน้ำตาผิดปกติ ทำให้น้ำตาระเหยไปเร็วขึ้น สาเหตุอื่นๆ เช่น abnormal eyelid position, decrease blink function, topical medication, chronic contact lens wear, ocular allergy syndromes
- Hx: มีอาการตาแห้ง ตาแดง เคืองตา แสบตา ตามัว สู้แสงไม่ได้ รู้สึกเหมือนมีเศษในตา น้ำตาไหลมาก (paradoxical excessive tearing)
- PE: prominent conjunctival vessels, abnormal corneal surface (neovascularization, opacity/scarring, loss of luster), blepharitis, eyelid malposition (entropion, ectropion), reduced blink rate (< 5/min)
- Tx:
- Artificial tears ให้ทุกราย โดยเฉพาะในรายที่มี ocular sensation ผิดปกติ (เช่น diabetic neuropathy) เพื่อป้องกัน ocular damage แนะนำให้เริ่มจาก eye drop QID ถ้ายังมีอาการอาจใช้ถี่ขึ้น แต่ถ้าใช้ > 4-6 ครั้ง/วัน แนะนำชนิดที่เป็น preservative-free
- Environmental strategies ได้แก่ ให้กระพริบตาบ่อยขึ้นโดยเฉพาะช่วงที่ต้องใช้สายตา หลีกเลี่ยงเครื่องปรับอากาศหรือเครื่องทำความร้อน
ใช้เครื่องเพิ่มความชื้นในอากาศ ใช้ moisture chamber glasses เพื่อรักษาความชื้นรอบดวงตา

- รักษาสาเหตุ เช่น แก้ไข eyelid abnormality, หยุดยาที่เป็นสาเหตุ, รักษา blepharitis (warm compression +/- topical ATB)
- Refer ophthalmologist ในรายที่มี neuropathy, uncertain Dx, severe pain, visual loss, abnormal blinking, incomplete eye closure
- ทางเลือกการรักษาอื่นๆ ได้แก่ topical cyclosporine, topical lifitegrast, scleral contact lenses, punctal occlusion, autologous serum tears, tear stimulation, oral antioxidants (vitamin E, B2, C, zinc, selenium), acupuncture, surgery
