 Premature ventricular complex

• PVC พบได้เป็นปกติในประชากรทั่วไป (80%) ค่าปกติ คือ < 500/24 ชั่วโมง โดยพบเพิ่มขึ้นตามอายุ และปัจจัยอื่นๆ เช่น faster sinus rate, hypokalemia, hypomagnesemia, และ hypertension
• S&S: ส่วนใหญ่ไม่มีอาการ อาการที่พบบ่อยที่สุดคือ อาการใจสั่น เกิดจาก post-PVC pause หรือ post-PVC beat hypercontraction โดยปกติ PVC จะไม่ทำให้เกิด hemodynamic compromise ยกเว้นว่ามี severely depressed LVEF หรือมี underlying bradycardia
• Typical ECG finding: QRS > 120 ms, QRS morphology ไม่เหมือน typical LBBB หรือ RBBB, opposite T wave direction, compensate pause (PP interval ยาว 2 เท่า)
• No investigation ถ้าสุขภาพโดยทั่วไปดี ไม่มีอาการและไม่มีประวัติโรคหัวใจในครอบครัว ตรวจร่างกายปกติ ECG ปกติ เป็น unifocal PVC < 500/24h และไม่มี exertional PVCs
• Ix (ขึ้นกับ Hx, PE): electrolytes (+ K, Mg), CBC, TSH, BNP, ABG, drug screen (cocaine, amphetamine), digoxin level
• Refer: รายที่มีหรือสงสัยโรคหัวใจ มีอาการผิดปกติ มี high PVC burden (> 10%) แนะนำให้ส่งตัวพบ cardiologist เพื่อตรวจเพิ่มเติม (ambulatory ECG monitoring, lab studies, echocardiography, EST, cardiac MRI)
• Risk for PVC-induced cardiomyopathy เช่น PVC burden > 10%, boarder PVC, interpolated PVC (RR interval ไม่เปลี่ยน), epicardial PVC origin (pseudo-delta wave > 50 ms, shortest RS > 115 ms, Q wave in aVL)
• Risk for VT/ VF ได้แก่ Purkinje PVC origin (short coupled PVC (PVC เกิดหลัง QRS < 300 ms [R on T]); Right Purkinje origin มักเป็น wide QRS + LBBB/LAD + late precordial transition > V4; Left Purkinje มักเป็น RBBB + relatively narrow QRS + RAD [anterior Purkinje] หรือ LAD [posterior Purkinje]), Malignant RVOTLVOT PVC (กลุ่มนี้จะแยกยากกว่า เพราะ CI > 300 จะสงสัยถ้า prematurity index < 0.73 [coupling interval/R-R interval ของ sinus cycle ก่อนหน้า])

• ABC-VT risk score มีการนำมาใช้เพื่อประเมินความเสี่ยง (reduced LVEF, HF, cardiac death); score < 4 = low risk

• ในรายที่ไม่มีอาการ และ PVC burden < 10% ไม่ต้องรักษา หรือถ้ามี structural heart disease ให้เน้นรักษาโรคเดิม ซึ่งมักมี beta-blocker ที่สามารถลดจำนวน PVC ลงได้
• ในรายที่มีอาการ ให้รักษาสาเหตุก่อน (alcohol, caffeine, illicit drugs, thyroid, electrolyte) ให้เลือกยาในกลุ่ม beta-blocker หรือรองมาคือ CCB ก่อน ในรายที่รักษาไม่ได้ผลให้พิจารณาทำ catheter ablation หรือ long term antiarrhythmic therapy (ในกรณีที่ไม่สามารถทำ ablation ได้  ถ้ามี structural heart ให้ amiodarone หรือถ้าไม่มี structural heart ให้ flecainide, propafenone)
• Metoprolol 25 mg PO BID (rarely > 200 mg/d)
• Carvedilol 3.125 mg PO BID (max 25 mg BID)
• Bisoprolol 2.5 mg PO OD (max 10 mg OD)
• Atenolol 25 mg PO OD (rarely > 100 mg/d)
• Propanolol 10 mg PO BID-TID (max 320 mg/d; DOC ใน pediatric, some channelopathy, hyperthyroidism)
• Diltiazem 60 mg PO BID (max 480 mg/d)
• Verapamil 80 mg PO TID (max 480 mg/d)
• ในรายที่มี high PVC burden ร่วมกับมี PVC-induced cardiomyopathy + LVEF < 50% ถ้าไม่มีอาการให้ beta-blocker หรือ CCB ก่อน แต่ถ้าไม่ดีขึ้นหรือมี HF ให้ทำ radiofrequency catheter ablation ร่วมกับให้ beta-blocker
